4th and 5th Grade Center Clinic
Welcome to the Fourth and Fifth Grade Clinic, my name is Brenda Douglas CNA/RMA. My office hours are from 7:00 a.m.-3:30 p.m. and I'm available to answer questions or address concerns you may have about your student and their health needs at school. Please refer to the Parent/Student Handbook for medication guidelines and procedures or if you have individual questions, please contact me at 352-259-6850 or you may also send an email to brenda.douglas@tvcs.org.
Our school year is coming to an end, so here is a quick reminder to those parents who have students with medications in the clinic.
Medications in the clinic will need to be picked up by May 20th 2022. Medication not picked up by 05/20/22 will be destroyed.
When you pick up your student’s medication you will also receive the paperwork you will need for the next school year if your student will continue to have medicine needs. The paperwork is based on your student’s current medication for the 2021-2022 school year. If you need additional forms you can print them from the VCS website. Go to TVS. org select Elementary, then Clinic and then 4-5 grade center.
On Orientation Day bring the completed forms and the medication to the clinic to sign in by parent and to update care plans, if needed. Things will go smoothly when everything is completed ahead of time.
Please note the following:
* All physician prescriptions for school medications are good for one school year only
*Must use Sumter County Medication Authorization forms.
*The prescription label must match the Medication Authorization form.
*Over the counter medications must be new/unopened.
*Seizure, Asthma and Allergy Healthcare plans and Emergency Action Plans are done every 3 years or sooner if major changes have been made.
*If your child has an inhaler, please consider asking your child’s doctor about a spacer with a mask.
